Cancellation Policy

Welcome to MyTraitRx. Because our practice is led by a licensed Doctor of Pharmacy (PharmD) providing highly customized, data-backed multi-omic analyses, our clinical time is strictly managed. By scheduling an appointment, completing a screening form, or purchasing a package, you explicitly agree to the following terms: 1. Mandatory Screening Requirement To protect the legal and geographic integrity of our practice, every client must complete our 3-Question Eligibility Questionnaire before a 15-minute Strategy Audit or full consultation slot is officially secured. MyTraitRx reserves the right to automatically cancel any reservation that fails to complete this form within 24 hours of booking. 2. Rescheduling & Cancellation Windows * More than 48 Hours Notice: You may freely reschedule your live video consultation or cancel your appointment through your secure MyTraitRx client dashboard with zero penalty. * Less than 48 Hours Notice: Appointments rescheduled or canceled within 48 hours of the scheduled start time are subject to a $75 short-notice fee to cover lost clinical availability. * No-Shows: If you miss your scheduled video link completely or arrive more than 15 minutes late without prior written notice, the session is forfeited. A $100 re-booking fee will apply to reactivate your consultation slot. 3. Advanced Laboratory Kit Exceptions Once an advanced testing kit has been formally ordered and drop-shipped to your home address, the laboratory processing fee and kit is 100% non-refundable. You will retain possession of your genetic testing kit. 4. HSA / FSA Card Processing MyTraitRx confidently accepts HSA and FSA debit cards directly at checkout. An itemized receipt containing professional clinical line items and credentials will be generated automatically for your tax files. While our services are widely classified as eligible expenses, individual plan administrators retain final authority on coverage. It remains the client’s sole responsibility to confirm out-of-state genomic wellness eligibility with their personal provider. 5. Geographic Scope & Care Delivery * Clinical In-State Track: Full prescription audits, laboratory diagnostics, and physician co-management are strictly limited to individuals physically located within our active licensed jurisdictions (Kentucky, Arizona, Indiana, and Florida) at the time of service. * Nationwide Consulting Track: All reviews will focus strictly on baseline nutritional genomics and lifestyle architecture
